What is Anticipatory Care?
Anticipatory care is a proactive approach to health that focuses on the management of long-term conditions, including diabetes, heart disease, musculoskeletal disorders, recurrent infections and mental health issues. The aim is to identify potential health risks early and implement personalised care plans tailored to an individual’s specific needs and preferences. This approach seeks to improve overall health and wellbeing, enhance quality of life, and support patients in living as independently as possible.
The Anticipatory Care Team
The Anticipatory Care Team consists of a GP, paramedic practitioner, prescribing pharmacist and care coordinator. The team works alongside GP practices across Central Camden PCN to proactively identify and support elderly, frail and housebound patients, helping to coordinate and deliver anticipatory, holistic care in the community.
What will the Anticipatory Care Team do?
The Anticipatory Care Team provides proactive, home-based support for non-ambulatory patients, delivering comprehensive assessments using a holistic approach. The team considers medical, social, psychological and functional needs to build a full understanding of each individual’s circumstances.
A key element of the service is anticipatory care planning. This includes identifying what matters most to patients in relation to their health and wellbeing and, where appropriate, supporting early conversations about future care planning.
Where possible, the team encourages the involvement of family members or regular carers during visits to ensure carer needs and concerns are also identified and addressed.
The service is specifically focused on the management and coordination of care for people with long-term conditions. It does not provide support for acute or urgent clinical issues. Patients requiring urgent care are advised to contact their registered GP practice via usual access routes or NHS 111.
